Your Video Conference (Telehealth) Appointment
Your appointment will be held via online video conference (telehealth).
It is NOT a phone call.
Attend using a computer, tablet, or smartphone with internet, microphone, and camera.

The patient must be present during the virtual appointment.
This is a requirement for all telehealth visits. The doctor will need to interact directly with the patient during the appointment.

Complete your intake form if you are:
- A new patient
- A returning patient not seen in 6+ months
If your intake form is not submitted 5 days before your appointment, it may be rescheduled as the doctor uses it to prepare.

Review cancellation and missed appointment policies
Missed or canceled appointments within 2 business days incur a $75 fee. Immediate rebooking may not be possible as we typically book 2 months in advance.
